Contract completed and signed by yourself

notes (anonymised)

party information (such as family members or other parties if relevant)

data (including details about payments to and from you and details of products
and services you have purchased)
· Usage
data (including information about how you interact with and use our website,
products and services)
· Health
information (such as medical records or health conditions)
· Website

We also collect or use the
following special category information to provide and improve products and
services for clients. This information is subject to additional protection due
to its sensitive nature:

personal information for dealing with queries, complaints or claims:

This is to be able to contact you
when we need to, for example, to book, cancel or discuss your appointments, or
in the event of an emergency.
Lawful bases and data protection rights

UK data protection law, we must have a “lawful basis” for collecting and using

In line with my insurance
provider, I am obliged to keep records for five years after you stop
interacting with the business. This includes keeping your contact details.
Process notes are kept for upto two months and are then disposed of securely.
